We understand you may feel anxious about visiting the dentist’s office Whether you were referred to us for specific treatment, or merely looking for your primary care dentist. As our mission statement states, we are sensitive to your needs not only dentally but emotionally as well. We would like you to feel like you are at home in good hands.

YOUR FIRST VISIT IS ALL ABOUT YOU!   Dr. Chung will a spend good amount of time to get to know you better. You can express your feelings or anxiety with your previous dental experience. We want to know about your medical history, family history and everything you want to share with us. We do appreciate your active participation in this process. We can better serve you when we know more about you.

He will also explain to you all about us, share his philosophy and the goal we aim for. You can ask any questions about the practice, the doctor and other related policies and. We will answer all your questions to the best of our knowledge.

Comprehensive oral evaluation is given as the next step. It includes extra-, intraoral cancer check, lymph nodes check, masticatory muscles and joint palpation, existing restoration recording and current status assessment. Often full periodontal charting is done at the same visit. Full mouth radiographs may be obtained to help us make an accurate diagnosis. If you have radiographs taken from your previous dentist’s office, please have them transferred to our office in advance.

We respect your time schedule. Please prepare about an hour for your first visit with Dr. Chung.

 

Please assist us by providing the following information at the time of your appointment if available:

  • Your referral slip and any x-rays and periodontal charting if applicable (unless your dentist has sent these to us directly).
  • A list of medications you are currently taking (especially those taking blood thinners or bisphosphonates).
  • If you have dental insurance, bring your card and the necessary completed forms.

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them via email (info@seattleimplantdds.com) or mail (see map and location for address) to our office. If there is not enough time, please pick them up and bring them to our office for your initial exam. If additional films are necessary, they can be taken at our facility.
